COVID-19 Tuition Appeal

Please complete this form in its entirety. This is the first step to determine eligibility. If this is your first time applying for help, please also submit the Application for Flexible Tuition. Instructions are available under the Flexible Tuition tab on our website.

2. Submit copies of the following Covid-19 family impact documentation to the Flexible Tuition Office as applicable:
a. 2019 IRS Form 1040 with all attached schedules
b. 2019 W-2 or IRS Form 1099
c. Last Paycheck Stub for those that lost their job/reduced income due to COVID-19
d. Unemployment award letter and/or furlough/lay-off/fired letter from employer
e. Reduced income documentation from your employer
f. Current Balance Sheet/P & L for those that business has closed due to COVID-19

Separated/Divorced/Never Married Parents: It is the school’s policy to consider both parents’ financial resources when determining the tuition appeal. Both parents must submit all the items listed above.
